Uses GEMCAL NASAL SPRAY is prescribed for several medical conditions, primarily affecting bone metabolism:
- Postmenopausal Osteoporosis: Treatment of established postmenopausal osteoporosis to prevent bone breakdown and increase bone density, reducing the risk of fractures. It is particularly indicated for women more than five years postmenopause when alternative therapies are unsuitable.
- Bone Pain: Associated with osteolysis and/or osteopenia.
- Paget's Disease of Bone (Osteitis Deformans): A chronic disorder of bone remodeling.
- Neurodystrophic Disorders: Also known as algodystrophy or Sudeck's disease, arising from various factors such as post-traumatic painful osteoporosis, reflex dystrophy, shoulder-arm syndrome, causalgia, and drug-induced neurotrophic disorders.
Dosage and Administration GEMCAL NASAL SPRAY is for intranasal use only, delivering 200 IU calcitonin (salmon) per actuation.
- Priming the Pump: Before the first dose, the bottle should be at room temperature. Hold the bottle upright and depress the two white side arms of the pump until a full spray is produced. The pump is primed once the first full spray is emitted. The pump only needs to be primed before the first use.
- Administration: Carefully place the nozzle into the nostril with the head in an upright position and firmly depress the pump toward the bottle. Alternate nostrils daily. Wipe the nozzle with a clean tissue and replace the cap after each use. Do not shake the bottle before use.
- Postmenopausal Osteoporosis: Recommended dosage is 200 IU once a day intranasally, alternating nostrils daily. It should be used in conjunction with adequate calcium (at least 1000 mg elemental calcium) and Vitamin D (400 IU per day) intake.
- Bone Pain Associated with Osteolysis and/or Osteopenia: 200-400 IU daily. Up to 200 IU can be given as a single dose; higher dosages should be divided. Dosage should be adjusted to individual needs.
- Paget's Disease: 200 IU daily as a single dose. In some cases, 400 IU in divided doses may be necessary initially. Treatment should continue for at least 3 months or longer as required.
- Neurodystrophic Disorders: 200 IU daily in a single dose for 2-4 weeks. An additional 200 IU may be administered every second day for up to 6 weeks depending on clinical progress.
Side Effects Common side effects include:
- Nasal adverse reactions: Runny nose, nosebleeds (epistaxis), rhinitis (stuffy nose), nasal irritation, dryness, itching, or discomfort.
- Headache.
- Musculoskeletal pain: Back pain, joint pain (arthralgia), muscle aches, and muscle cramps.
- Hypocalcemia (low blood calcium levels).
- Transient dizziness, which may impair reaction.
Serious side effects, though rare, can include:
- Serious allergic/hypersensitivity reactions, including anaphylaxis and anaphylactic shock.
- Swelling of the face, lips, tongue, or throat.
- Bronchospasm.
Precautions
- Hypersensitivity: Patients with suspected hypersensitivity to calcitonin salmon should be considered for skin testing prior to treatment. Discontinue use immediately if a rash, itchy skin, swelling of the face and mouth, or difficulty breathing occurs.
- Hypocalcemia: Hypocalcemia must be corrected before initiating therapy. Other disorders affecting mineral metabolism (e.g., Vitamin D deficiency) should also be treated. Serum calcium and symptoms of hypocalcemia should be monitored during therapy.
- Calcium and Vitamin D Intake: Adequate intake of calcium and Vitamin D is recommended to prevent progressive loss of bone mass and enhance treatment effectiveness.
- Nasal Examinations: Periodic nasal examinations with visualization of the nasal mucosa, turbinates, septum, and mucosal blood vessels are recommended prior to, periodically during, and at any time nasal symptoms occur, due to potential mucosal alterations.
- Dizziness: GEMCAL NASAL SPRAY may cause transient dizziness. Patients should be warned not to drive or use machines if dizziness occurs.
- Drug Interactions:
- Concomitant use with lithium may cause a decrease in blood lithium levels due to increased urinary excretion.
- Digoxin (antiarrhythmic medicine) toxicity may increase.
- Bisphosphonates for osteoporosis might increase the rate of bone absorption.
- Pregnancy and Breastfeeding: Not generally recommended during pregnancy or breastfeeding; consult a doctor.
- Children: Not appropriate for pediatric use as it is indicated for postmenopausal women.
- Underlying Conditions: Inform your doctor if you have low blood calcium levels or any form of cancer. Contraindicated in individuals with hypersensitivity to synthetic calcitonin (salmon) or any excipients.
Storage
- Unopened Bottle: Store in a refrigerator at a temperature between 2°C to 8°C. Protect from freezing.
- Opened Bottle: Once opened, the bottle may be stored at room temperature below 25°C (some sources specify between 15°C and 30°C) in an upright position for up to 4 weeks (some sources say up to 30 or 35 days). Frequently asked questions
Frequently Asked Questions about GEMCAL NASAL SPRAY 3.7 ML
What is Gemcal Nasal Spray used for?
Gemcal Nasal Spray is primarily used to treat postmenopausal osteoporosis, a condition that weakens bones and increases the risk of fractures in women who have completed menopause. It may also be used to manage bone pain associated with Paget's disease and other bone disorders.
How does Gemcal Nasal Spray work?
Gemcal Nasal Spray contains Calcitonin (salmon), a hormone that helps regulate calcium levels in the body. It works by suppressing the activity of osteoclasts, which are cells responsible for breaking down bone tissue. By inhibiting bone breakdown, it helps to increase bone density and strengthen bones, thereby reducing the risk of fractures.
What is the usual dosage of Gemcal Nasal Spray?
The typical recommended dosage for postmenopausal osteoporosis is 200 International Units (IU) administered once a day intranasally, with alternating nostrils daily. It is crucial to use this medicine exactly as prescribed by your doctor. Your doctor may also recommend adequate calcium (at least 1000 mg elemental calcium) and Vitamin D (400 IU per day) supplements to be taken concurrently to enhance treatment effectiveness.
What are the possible side effects of Gemcal Nasal Spray?
Common side effects include a runny nose, nosebleeds, nasal irritation, stuffy nose, back pain, headache, muscle pain, joint pain, and sinus pain. Some individuals may experience hypocalcemia (low blood calcium levels). More serious, but rare, side effects can include severe allergic reactions (like anaphylactic shock) or severe hypocalcemia, which might manifest as muscle cramps or tingling sensations. If side effects persist or worsen, or if you notice any ulceration inside your nose, consult your doctor.
Is Gemcal Nasal Spray safe during pregnancy or breastfeeding?
Gemcal Nasal Spray is not recommended for use during pregnancy as there is limited information, and animal studies have shown harmful effects on the developing baby. It is also generally considered unsafe during breastfeeding as limited human data suggests the drug may pass into breast milk. This medication is intended for postmenopausal women.
Can Gemcal Nasal Spray be taken with other medications?
Gemcal Nasal Spray can interact with certain medications. For example, it may decrease the blood level of lithium (used for depression and mania) and can increase the toxicity of antiarrhythmic medicines like digoxin. It may also interact with other osteoporosis medications. Always inform your doctor about all prescription, over-the-counter medications, and herbal supplements you are currently taking to avoid potential drug interactions.
What precautions should be taken while using Gemcal Nasal Spray?
Gemcal Nasal Spray is for intranasal use only and should not be injected or taken orally. Inform your doctor about any pre-existing medical conditions, especially low blood calcium levels (hypocalcemia) or any form of cancer. It is important to maintain an adequate intake of calcium and Vitamin D, as advised by your doctor. Avoid alcohol consumption, as it can affect calcium and Vitamin D absorption. This spray may cause temporary dizziness or fatigue, so avoid driving or operating machinery until you know how it affects you. Regular nasal examinations may be recommended to monitor for nasal irritation or ulceration.
What happens if a dose of Gemcal Nasal Spray is missed?
If you miss a dose, use it as soon as you remember. However, if it is almost time for your next scheduled dose, skip the missed dose and continue with your regular dosing schedule. Do not double the dose to make up for a missed one.
How should Gemcal Nasal Spray be stored?
Unopened bottles of Gemcal Nasal Spray should be stored in a refrigerator at a temperature between 2°C to 8°C (36°F to 46°F) and protected from freezing. Once opened, the bottle can be stored at room temperature (15°C to 30°C or below 25°C) in an upright position for up to 35 days (or 4 weeks). Allow the spray to reach room temperature before use. Always keep this medication out of the reach and sight of children.
