Description
TOBAMIST RESPULES by CIPLA-RESP NEB 1 DIV
Name
Common Name: Tobramycin Inhalation Solution Brand Name: TOBAMIST RESPULES
Composition
TOBAMIST RESPULES contain Tobramycin as the active ingredient. Each 5 mL respule typically contains 300 mg of Tobramycin in an isotonic solution.
Uses
TOBAMIST RESPULES is an aminoglycoside antibiotic primarily used for the management of chronic chest infections caused by the bacterium Pseudomonas aeruginosa in patients with cystic fibrosis. It helps combat serious lung infections by inhibiting bacterial protein synthesis, thereby reducing respiratory symptoms like cough and breathlessness. It is also used in the management of bronchiectasis with bacterial involvement.
Dosage and Administration
The recommended dosage for adults and pediatric patients aged 6 years and older is one single-use respule (300 mg/5 mL) administered twice daily by oral inhalation using a nebulizer. Doses should be taken as close to 12 hours apart as possible, but not less than 6 hours apart. The treatment follows an alternating cycle of 28 days on the drug, followed by 28 days off the drug. The solution is not for injection or swallowing.
Side Effects
Common side effects include runny or stuffy nose, sneezing, voice alteration (hoarseness), discoloration of sputum, worsening of lung function test results, feeling unwell, muscle pain, cough, sore throat, taste change, and increased sputum production. More serious side effects can include hearing loss, ringing or buzzing in the ears (tinnitus), dizziness, and potential effects on kidney function.
Precautions
TOBAMIST RESPULES should not be used if you have a known hypersensitivity to tobramycin or any other aminoglycoside antibiotics. Caution is advised in patients with hearing problems, kidney problems, neuromuscular disorders such as myasthenia gravis or Parkinson's disease, or unusual difficulty breathing with wheezing or coughing. It is not recommended for children under six years of age. Pregnant or breastfeeding women should consult their doctor before use. Monitoring of kidney function may be recommended during treatment.
Storage
TOBAMIST RESPULES should be stored under refrigeration at 2-8°C (36-46°F). If refrigeration is unavailable, the respules (opened or unopened pouches) can be stored at room temperature (up to 25°C/77°F) for up to 28 days. The solution should not be exposed to intense light. While the solution may darken with age if not refrigerated, this color change does not indicate a change in product quality if stored as recommended. Keep out of sight and reach of children.
Frequently asked questions
What is TOBAMIST RESPULES used for?
TOBAMIST RESPULES, which contains tobramycin inhalation solution, is an antibiotic primarily used for the management and prevention of lung infections caused by the bacterium Pseudomonas aeruginosa in patients aged 6 years and older with cystic fibrosis (CF). These infections are common in CF patients due to thick, sticky mucus in their lungs.
How does TOBAMIST RESPULES work?
TOBAMIST RESPULES contains tobramycin, an aminoglycoside antibiotic. When inhaled, it directly reaches the lungs and works by disrupting protein synthesis in bacteria, which leads to altered cell membrane permeability, disruption of the cell envelope, and ultimately, bacterial cell death. This action helps to kill or stop the growth of the infection-causing bacteria.
What is the usual dosage of TOBAMIST RESPULES?
The typical dosage for adults and children aged 6 years and older is one 300 mg/5 mL ampule inhaled twice daily for 28 days. This is followed by a 28-day period off the medication, and then the cycle is repeated (28 days on, 28 days off). Doses should be taken as close to 12 hours apart as possible, but not less than 6 hours apart.
What are the possible side effects of TOBAMIST RESPULES?
Common side effects can include cough, sore throat, changes in voice (hoarseness), and altered taste. More serious side effects, which should be reported to a healthcare provider immediately, include hearing loss or ringing in the ears (ototoxicity), worsening kidney problems (nephrotoxicity), worsening muscle weakness (especially in patients with neuromuscular disorders), and severe breathing problems like bronchospasm.
Is TOBAMIST RESPULES safe during pregnancy or breastfeeding?
TOBAMIST RESPULES should generally not be used during pregnancy or breastfeeding unless the potential benefits to the mother outweigh the potential risks to the fetus or baby. Aminoglycosides can cause fetal harm, such as congenital deafness, if high systemic concentrations are achieved in a pregnant woman. It is not known if inhaled tobramycin passes into breast milk, but if used while breastfeeding, the infant should be monitored for diarrhea or blood in the stool.
Can TOBAMIST RESPULES be taken with other medications?
Concurrent and/or sequential use of TOBAMIST RESPULES with other medications that have neurotoxic, nephrotoxic, or ototoxic potential should be avoided. Certain diuretics, such as ethacrynic acid, furosemide, urea, or intravenous mannitol, should also not be administered concomitantly as they can enhance aminoglycoside toxicity. Importantly, TOBAMIST RESPULES should not be mixed or diluted with other medications in the nebulizer.
What precautions should be taken when using TOBAMIST RESPULES?
Caution should be exercised in patients with known or suspected kidney, auditory, vestibular, or neuromuscular disorders (e.g., Parkinson's disease, myasthenia gravis), as aminoglycosides may aggravate muscle weakness. Bronchospasm can occur, particularly with the first dose, and patients should be monitored. Regular monitoring of kidney function and serum tobramycin concentrations may be recommended.
What happens if a dose of TOBAMIST RESPULES is missed?
If a dose is missed, it should be taken as soon as possible, provided there are at least 6 hours remaining before the next scheduled dose. If there are less than 6 hours until the next dose, the missed dose should be skipped, and the patient should continue with their regular dosing schedule. Do not double doses to make up for a missed one.
How should TOBAMIST RESPULES be stored?
TOBAMIST RESPULES should be stored under refrigeration, typically between 2-8°C (36-46°F). If refrigeration is unavailable, the solution pouches (opened or unopened) may be stored at room temperature (up to 25°C / 77°F) for a maximum of 28 days. It should be protected from intense light. Do not use the medication if it is cloudy, contains particles, or has passed its expiration date.
