Description

MACOX PLUS

MACOX PLUS is a combination medication manufactured by MACLEODS PHARMACEUTICALS LTD., primarily used for the treatment of tuberculosis (TB).

Composition

MACOX PLUS typically contains a combination of two antitubercular agents:

  • Rifampicin: 450 mg (also available in 150 mg and 600 mg strengths in other formulations)
  • Isoniazid: 300 mg (also available in 100 mg strengths in other formulations)

These active ingredients work synergistically to eliminate and prevent the growth of Mycobacterium tuberculosis, the bacteria responsible for TB.

Uses

MACOX PLUS is prescribed for the treatment of:

  • Tuberculosis (TB): A bacterial infection predominantly affecting the lungs, but capable of spreading to other body parts like kidneys, brain, spine, and bone marrow.
  • Bacterial infections of membranes covering the brain and spinal cord.
  • Contagious diseases of the skin, nerves, and mucous membranes.
  • Preventive therapy in high-risk pediatric contacts of TB-positive individuals.

Dosage and Administration

MACOX PLUS should be taken as directed by a healthcare professional. It is generally advised to take the tablet on an empty stomach, either one hour before or two hours after meals, at a fixed time each day for optimal results. The tablet should be swallowed whole and not chewed, crushed, or broken. It is crucial to complete the entire prescribed course of treatment, even if symptoms improve, to prevent recurrence or worsening of the infection and to avoid drug resistance.

Side Effects

Common side effects may include:

  • Nausea, vomiting, diarrhea, stomach pain, loss of appetite, heartburn.
  • Fever, sweating, increased saliva.
  • Headache, dizziness, unusual tiredness or weakness.
  • Peripheral neuropathy (tingling and numbness in hands and feet).
  • Dark-colored urine, reddish-orange discoloration of body fluids (urine, sweat, saliva, tears); this is a normal and harmless effect.

Serious, though rare, side effects that require immediate medical attention include:

  • Allergic reactions such as rash, itching, swelling, or difficulty breathing.
  • Signs of liver damage like jaundice (yellowing of skin or eyes), persistent fatigue, dark urine, or abdominal pain.
  • Vision changes, convulsions, depression, or joint pain.

Precautions

  • Liver and Kidney Impairment: Patients with pre-existing liver or kidney conditions should use MACOX PLUS with caution and under strict medical guidance, as dosage adjustments may be necessary. Severe liver impairment is a contraindication.
  • Drug Interactions: Inform your doctor about all other medications, supplements, and herbal remedies you are taking. MACOX PLUS can reduce the effectiveness of hormonal birth control, requiring the use of alternative contraceptive methods. It can also interact with antiviral medications, certain antifungal, anti-anxiety, Parkinson's, epilepsy, HIV, and alcohol addiction medications.
  • Alcohol Consumption: Avoid alcohol intake during treatment as it increases the risk of liver damage.
  • Food Interactions: Avoid tyramine-rich foods such as cheese, smoked fish, meats, and certain types of beer while on this medication.
  • Pregnancy and Breastfeeding: Pregnant or breastfeeding women should consult their doctor before taking this medication.
  • Driving: Refrain from driving or operating heavy machinery if you experience drowsiness or dizziness.
  • Contact Lenses: MACOX PLUS may permanently stain soft contact lenses due to body fluid discoloration.

Storage

Store MACOX PLUS in its original container, tightly closed, in a cool and dry place, away from direct sunlight. The recommended storage temperature is below 30°C (or between 10°C to 25°C). Keep the medicine out of the sight and reach of children. Dispose of any unused medication responsibly as per local guidelines.

Frequently asked questions

What is Macox Plus used for?

Macox Plus is a combination medication primarily prescribed for the treatment of tuberculosis (TB). Tuberculosis is an infectious disease that commonly affects the lungs but can also spread to other parts of the body, such as the kidneys, brain, spine, and bone marrow. It is also used in some cases for the prevention of TB in individuals who have been in close contact with TB patients.

How does Macox Plus work?

Macox Plus contains two active ingredients: Rifampicin and Isoniazid. Rifampicin works by inhibiting a bacterial enzyme called DNA-dependent RNA polymerase, which is essential for the tuberculosis bacteria to make proteins and reproduce. Isoniazid prevents the formation of mycolic acid, a major component of the bacterial cell wall, thereby hindering the growth of the bacteria. Together, these two antibiotics kill the Mycobacterium tuberculosis bacteria and help eradicate the infection.

What is the usual dosage of Macox Plus?

The dosage and duration of Macox Plus vary depending on the patient's condition, age, and severity of the disease, and must be strictly followed as prescribed by a doctor. It is typically taken once daily on an empty stomach, preferably one hour before or two hours after meals, at a fixed time each day to ensure better efficacy. The tablet should be swallowed whole and not chewed, crushed, or broken. It is crucial to complete the full course of treatment, even if symptoms improve, to prevent reinfection and the development of drug-resistant TB.

What are the possible side effects of Macox Plus?

Common side effects of Macox Plus include nausea, vomiting, fever, dark-colored urine, sweating, increased saliva, rash, and peripheral neuropathy (tingling and numbness in the hands and feet). More serious but rare side effects can include jaundice, increased liver enzymes, and allergic reactions like itching, swelling, or difficulty breathing. If any side effects worsen or severe reactions occur, immediate medical attention should be sought.

Is Macox Plus safe during pregnancy or breastfeeding?

Macox Plus Tablet may be unsafe to use during pregnancy as animal studies have shown harmful effects on the developing baby, and there are limited human studies. It is advised to consult your doctor before taking this medication if you are pregnant or planning a pregnancy. For breastfeeding women, Macox Plus is generally considered safe if prescribed by a doctor, as human studies suggest the drug does not pass into breast milk in significant amounts and is not harmful to the baby. However, it is important to consult your doctor.

Can Macox Plus be taken with other medications?

Macox Plus can interact with several medications. It may reduce the effectiveness of medicines used for fungal infections (e.g., ketoconazole), anxiety (e.g., diazepam), and Parkinson's disease (e.g., levodopa). It can also increase the toxicity and adverse effects of medicines for epilepsy (e.g., carbamazepine, phenytoin), HIV (e.g., stavudine), and alcohol addiction (e.g., disulfiram). Additionally, it can decrease the effectiveness of hormonal birth control, so alternative or additional contraceptive methods should be used. Always inform your doctor about all medications, supplements, and herbal remedies you are currently taking to avoid potential interactions.

What precautions should be taken while using Macox Plus?

Several precautions should be observed while taking Macox Plus:

  • Alcohol: Avoid alcohol consumption as it can increase the risk of liver damage and may cause a disulfiram-like reaction (flushing, increased heart rate, nausea, chest pain, low blood pressure).
  • Liver/Kidney Issues: Use with caution in patients with liver or kidney diseases, and it is not recommended in cases of severe liver impairment. Regular monitoring of liver function tests is recommended.
  • Driving: Avoid driving or operating heavy machinery if you experience side effects like drowsiness, dizziness, eye pain, vision problems, or numbness/tingling in hands and feet, as these can affect your ability to drive.
  • Contact Lenses: The medication may permanently stain soft contact lenses and can cause reddish-orange discoloration of body fluids like urine, sweat, saliva, and tears. This discoloration is usually harmless.
  • Food Interactions: It is generally recommended to take Macox Plus on an empty stomach and avoid tyramine-rich foods (e.g., cheese, smoked fish, certain beers, red wine) as they can interact adversely.
  • Diabetes: Use with caution in diabetic patients.
  • Children: Macox Plus is generally not recommended for children under 15 years of age due to lack of safety and efficacy data, though specific pediatric formulations exist and should be prescribed under medical guidance.

What happens if a dose of Macox Plus is missed?

If you miss a dose of Macox Plus, take it as soon as you remember. However, if it is almost time for your next scheduled dose, skip the missed dose and continue with your regular dosing schedule. Do not take a double dose to make up for a missed one, as this may lead to an overdose.

How should Macox Plus be stored?

Macox Plus should be stored in its original container, tightly closed, in a dry place, away from sunlight, at room temperature (typically below 25°C or 30°C). It should be kept out of the sight and reach of children and pets. Unused medication should be disposed of responsibly as biomedical waste.
