APIDRA 100 IU/ML 3 ML CARTRIDGE (5 CARTRIDGE)

APIDRA 100 IU/ML 3 ML CARTRIDGE is a rapid-acting insulin analogue manufactured by Sanofi India Ltd., used to manage blood sugar levels in individuals with diabetes mellitus.

Composition

Active Ingredient: Each ml of the solution contains 100 Units of Insulin Glulisine (equivalent to 3.49 mg). Other Ingredients: Metacresol, sodium chloride, trometamol, polysorbate 20, concentrated hydrochloric acid, sodium hydroxide, and water for injections.

Uses

APIDRA is indicated for the treatment of diabetes mellitus in adults, adolescents, and children aged 6 years and older. It is used to improve glycemic control in both Type 1 and Type 2 diabetes mellitus, helping to normalize blood sugar levels after meals (postprandial blood sugar control) and reduce the risk of severe complications associated with diabetes. It stimulates glucose uptake in muscle and fat cells and inhibits glucose production in the liver.

Dosage and Administration

APIDRA 100 IU/ML is administered subcutaneously (under the skin) using a reusable pen. It should be injected shortly (0-15 minutes) before a meal or within 20 minutes after starting a meal. Common injection sites include the abdominal wall, thigh, or upper arm. It is crucial to rotate injection sites within the same region to prevent skin changes like lumps or thickening. The dosage should be individually adjusted by a doctor and is often used in combination with intermediate or long-acting insulin, or oral hypoglycemic agents. Intravenous administration is possible but should be carried out under medical supervision.

Side Effects

Common Side Effects: Hypoglycemia (low blood sugar), rash, injection site allergic reaction, weight gain, edema (tissue swelling or fluid overload), skin thickening at the injection site (lipodystrophy), and itching. Serious Side Effects: Severe allergic reactions (generalized rash, shortness of breath, trouble breathing, fast pulse, sweating, feeling faint, swelling of face/tongue/throat), severe hypoglycemia (can lead to brain damage or be life-threatening), hypokalemia (low blood potassium), and heart failure (especially when used with thiazolidinediones).

Precautions

  • Hypoglycemia: Patients should be aware of the symptoms of low blood sugar and carry sugary items.
  • Alcohol: Alcohol consumption may either increase or decrease blood glucose levels and should be avoided.
  • Kidney and Liver Issues: Dose adjustment may be necessary for patients with kidney or liver problems.
  • Pregnancy and Breastfeeding: Generally considered safe if prescribed during pregnancy, but consultation with a doctor is advised, as insulin doses may need adjustment. Information on breastfeeding is limited; consult a doctor.
  • Driving: Ability to concentrate or react may be reduced if blood sugar levels are too low or too high; caution is advised while driving.
  • Drug Interactions: Concomitant use with pioglitazone (a thiazolidinedione) and insulin has been reported to cause heart failure in some patients.
  • Travel: Consult a doctor before traveling to discuss insulin availability, storage, and timing of meals/insulin administration across time zones.

Storage

Unopened cartridges should be stored in a refrigerator at 2°C to 8°C. Do not freeze APIDRA, and discard if it has been frozen. Keep the cartridge in its outer carton to protect it from light. Once in use, cartridges can be stored at room temperature for up to 4 weeks (28 days) but should not be refrigerated after being placed in a reusable pen. Do not use the medicine if it does not appear clear and colorless or if it contains particles. Empty cartridges must never be reused.

Frequently asked questions

Apidra (insulin glulisine) 100 IU/ML is a rapid-acting insulin analog used to manage blood sugar levels in individuals with diabetes mellitus. Below are frequently asked questions about this medication.

What is Apidra used for?

Apidra is a rapid-acting insulin analog indicated to improve blood sugar control in adults and pediatric patients (6 years of age and older) with type 1 and type 2 diabetes mellitus. It is typically used to manage blood sugar spikes that occur after meals.

How does Apidra work?

Apidra (insulin glulisine) is a recombinant human insulin analog that works by stimulating peripheral glucose uptake by skeletal muscle and fat, and by inhibiting glucose production in the liver. This action helps to lower blood glucose levels. It has a more rapid onset of action (within 10-20 minutes) and a shorter duration of activity (about 4 hours) compared to regular human insulin, making it effective for post-meal blood sugar control.

What is the usual dosage of Apidra?

The dosage of Apidra must be individualized based on the patient's metabolic needs, blood glucose monitoring results, and glycemic control goals. It is typically administered by subcutaneous injection within 15 minutes before a meal or within 20 minutes after starting a meal. Dosage adjustments may be necessary with changes in physical activity, concomitant medications, meal patterns, or during illness.

What are the possible side effects of Apidra?

The most common side effect of Apidra, like all insulins, is hypoglycemia (low blood sugar), which can be life-threatening. Other common side effects include injection site reactions such as pain, redness, irritation, itching, and mild skin rash. Less common but serious side effects can include generalized allergic reactions (anaphylaxis) and hypokalemia (low potassium levels).

Is Apidra safe during pregnancy or breastfeeding?

For pregnant individuals, Apidra should be used only if the potential benefit outweighs the potential risk to the fetus, and careful monitoring of glucose control is essential. Insulin requirements often change during pregnancy. For breastfeeding mothers, Apidra is generally considered acceptable, but adjustments in insulin dose and diet may be required. It is unknown if insulin glulisine is excreted in human milk, but insulin generally does not pass into breast milk or is absorbed orally.

Can Apidra be taken with other medications?

Yes, Apidra can interact with many other medications, which may either increase or decrease its blood-glucose-lowering effect. Drugs that may increase the risk of hypoglycemia include other antidiabetic agents, ACE inhibitors, and salicylates. Conversely, corticosteroids, niacin, and some thyroid hormones may decrease the blood-glucose-lowering effect. Alcohol can also increase the risk of hypoglycemia. Always inform your doctor or pharmacist about all prescription, over-the-counter medications, and supplements you are taking.

What precautions should be taken when using Apidra?

Always check the insulin label before each injection to avoid medication errors. Never share injection pens or syringes with other people, even if the needle is changed, due to the risk of transmitting blood-borne pathogens. Rotate injection sites within the same region (e.g., abdomen, thigh, or upper arm) to reduce the risk of lipodystrophy (thickening or hollowing of the skin). Monitor blood glucose levels frequently, especially when changing insulin regimens or during illness. Use with caution if you have liver or kidney problems, as insulin requirements may be reduced.

What happens if a dose of Apidra is missed?

If you miss a dose of Apidra, do not take a double dose to make up for the forgotten dose. Since Apidra is used before or soon after meals, if it's only been a short time since the missed dose and you remember to take it, you may do so and then eat a meal within 15 minutes. However, if it's close to the time of your next scheduled dose, it's generally best to skip the missed dose and continue with your regular schedule. Always check your blood sugar levels and consult your doctor or diabetes care team for specific guidance on how to handle a missed dose, as individual circumstances vary.

How should Apidra be stored?

Unopened Apidra cartridges should be stored in a refrigerator at 2°C to 8°C (36°F-46°F) and protected from light. Do not freeze Apidra, and discard it if it has been frozen. Once opened or in-use, Apidra cartridges should not be refrigerated but should be kept at room temperature (below 25°C or 77°F) away from direct heat and light for a maximum of 28 days. After 28 days of first use, the opened cartridge must be discarded.
