Name

  • Common Name: Methylprednisolone sodium succinate
  • Brand Name: SOLU-MEDROL 125MG

Composition

SOLU-MEDROL 125MG contains Methylprednisolone sodium succinate as its active ingredient. This is a synthetic glucocorticoid, a type of steroid hormone. Each 2 mL (when mixed) contains methylprednisolone sodium succinate equivalent to 125 mg of methylprednisolone, along with 1.6 mg monobasic sodium phosphate anhydrous and 17.4 mg dibasic sodium phosphate dried. Uses

SOLU-MEDROL is an anti-inflammatory glucocorticoid primarily used for its potent anti-inflammatory and immunosuppressant effects. It is prescribed for various conditions, including:

  • Allergic States: Severe or incapacitating allergic conditions such as asthma, atopic dermatitis, drug hypersensitivity reactions.
  • Rheumatic Disorders: As adjunctive therapy for acute gouty arthritis, acute rheumatic carditis, rheumatoid arthritis (including juvenile rheumatoid arthritis), ankylosing spondylitis, psoriatic arthritis, dermatomyositis, temporal arteritis, polymyositis, and systemic lupus erythematosus.
  • Skin Diseases: Bullous dermatitis herpetiformis, exfoliative erythroderma, mycosis fungoides, pemphigus, severe erythema multiforme (Stevens-Johnson syndrome), eczema, and psoriasis.
  • Eye Conditions: Inflammation of the eyes, sympathetic ophthalmia, uveitis, and ocular inflammatory conditions.
  • Respiratory Diseases: Berylliosis, fulminating or disseminated pulmonary tuberculosis (with appropriate antituberculous chemotherapy), idiopathic eosinophilic pneumonias, and symptomatic sarcoidosis.
  • Blood Disorders: Various blood disorders.
  • Gastrointestinal Diseases: Diseases of the gut, such as ulcerative colitis.
  • Nervous System: Acute exacerbations of multiple sclerosis and cerebral edema.
  • Endocrine Disorders: Primary or secondary adrenocortical insufficiency, congenital adrenal hyperplasia, hypercalcemia associated with cancer, and nonsuppurative thyroiditis.
  • Other Uses: Organ transplantation, prevention of nausea and vomiting associated with cancer chemotherapy, and certain glandular conditions.

Dosage and Administration

SOLU-MEDROL can be administered by intravenous (IV) injection, intravenous infusion, or intramuscular (IM) injection, with IV injection being the preferred method for initial emergency use.

  • High-Dose Therapy: For acute, life-threatening conditions, the recommended dose is 30 mg/kg intravenously over at least 30 minutes, repeatable every 4 to 6 hours for 48 hours.
  • Standard Therapy: Initial doses typically range from 10 to 40 mg, administered over several minutes.
  • Pediatric Dosing: The initial dose for pediatric patients may vary from 0.11 to 1.6 mg/kg/day in three or four divided doses.
  • Reconstitution: Use only the accompanying diluent or Bacteriostatic Water For Injection with Benzyl Alcohol. The solution should be inspected for particulate matter and discoloration before administration.

Side Effects

Common side effects include weight gain/loss, increased/decreased appetite, muscle weakness, increased sweating, headache, dizziness, fatigue, nausea, vomiting, acne, and stomach pain/discomfort or diarrhea. Serious side effects can include bone weakness/fractures, poor wound healing, yellowing of skin/eyes, dark urine, severe headache, stomach pain, blurred/double vision, convulsions, chest pain, allergic reactions, chemical imbalances in blood and urine, pancreatitis, stomach bleeding, increased risk of infection, hormone changes, metabolic changes, increased blood pressure, or increased white blood cell count. Long-term use can lead to thinning of the skin, cataracts, glaucoma, and growth suppression in children.

Precautions

  • Infections: Corticosteroids suppress the immune system and increase the risk of infection. They can mask signs of infection and exacerbate existing or latent infections.
  • Hypersensitivity: Contraindicated in patients with systemic fungal infections and known hypersensitivity to methylprednisolone or its components. Some formulations contain lactose derived from cow's milk.
  • Vaccinations: Avoid live or live-attenuated vaccines during treatment with immunosuppressive doses of corticosteroids.
  • Cardiac/Renal Conditions: Caution is advised in patients with heart problems, high blood pressure, or kidney disease. Rapid IV administration of large doses can cause cardiac arrhythmias or cardiac arrest.
  • Diabetes: Can increase blood glucose levels and worsen pre-existing diabetes.
  • Gastrointestinal: Use with caution in patients with gastrointestinal diseases due to increased risk of bleeding.
  • Pediatric Use: Long-term treatment can affect growth and development in children and increase the risk of high intracranial pressure. Some formulations contain benzyl alcohol, which can be toxic to neonates.
  • Drug Interactions: Can interact with various medications including NSAIDs, anticoagulants, cyclosporine, estrogen, and ketoconazole. Grapefruit juice should be avoided.
  • Withdrawal: Do not discontinue abruptly after prolonged therapy to avoid severe withdrawal symptoms.

Storage

Unreconstituted product should be stored below 25°C (77°F) and protected from light. Reconstituted solutions should be used immediately. If not used immediately, in-use storage times and conditions are the responsibility of the user. Chemical and physical stability of the further diluted product has been demonstrated within 4 hours of preparation if stored below 25°C, or within 24 hours if stored at 2°C to 8°C. Reconstituted solutions (not further diluted) should be used within 48 hours if stored at controlled room temperature 20° to 25°C.

Frequently asked questions

SOLU-MEDROL 125MG by PFIZER LIMITED is an injectable corticosteroid medication containing methylprednisolone sodium succinate. It is used for its potent anti-inflammatory and immunosuppressive properties.

What is SOLU-MEDROL 125MG used for?

SOLU-MEDROL 125MG is used to treat a wide range of inflammatory and allergic conditions. These include severe allergic reactions, arthritis, blood disorders, certain cancers, eye conditions, skin/kidney/intestinal/lung diseases, and immune system disorders. It can also be used as adjunctive therapy in severe acute conditions such as asthma exacerbations and systemic lupus erythematosus. In specific cases, it may be used for COVID-19 in hospitalized patients needing oxygen or mechanical ventilation.

How does SOLU-MEDROL 125MG work?

SOLU-MEDROL (methylprednisolone) is a synthetic glucocorticoid that acts as a potent anti-inflammatory and immunosuppressant. It works by decreasing the immune system's response to various diseases, which helps reduce symptoms like swelling, pain, itching, and other allergic reactions. It inhibits the release of substances in the body that cause inflammation.

What is the usual dosage of SOLU-MEDROL 125MG?

The initial dosage of SOLU-MEDROL 125MG varies significantly depending on the clinical problem being treated and the patient's response. For severe, acute conditions, larger initial doses may be required, sometimes up to 500 mg. Doses up to 250 mg should be given intravenously over at least five minutes, and doses greater than 250 mg over at least 30 minutes. Subsequent doses can be given intravenously or intramuscularly based on the patient's condition.

What are the possible side effects of SOLU-MEDROL 125MG?

Common side effects can include nausea, vomiting, heartburn, headache, dizziness, trouble sleeping, appetite changes, increased sweating, acne, or pain/redness/swelling at the injection site. More serious side effects that require immediate medical attention include:

  • Musculoskeletal: Bone/joint pain, muscle weakness/pain, osteoporosis, slow wound healing, thinning skin.
  • Gastrointestinal: Stomach/abdominal pain, black/tarry stools, vomit that looks like coffee grounds (signs of stomach/intestinal bleeding).
  • Mental/Mood Changes: Mood swings, depression, agitation, confusion, or severe headache.
  • Cardiovascular: Fast/slow/irregular heartbeat, chest pain or pressure, swelling of the ankles/feet/hands, high blood pressure.
  • Infections: Increased susceptibility to infections, worsening of existing infections, or masking signs of infection.

Is SOLU-MEDROL 125MG safe during pregnancy or breastfeeding?

Pregnancy: SOLU-MEDROL should be used during pregnancy only when clearly needed, as it may rarely harm an unborn baby. Infants born to mothers who used corticosteroids for a long time during pregnancy may have hormone problems and should be carefully observed for signs of hypoadrenalism. It's important to discuss the risks and benefits with your doctor. Breastfeeding: The medication passes into breast milk but is unlikely to harm a nursing infant. However, some mothers have reported decreased milk supply with methylprednisolone. It is generally considered compatible with breastfeeding, especially at lower doses. If the dose is high (e.g., above 80mg), it may be advisable to wait a few hours after administration before nursing. Consult your doctor before breastfeeding.

Can SOLU-MEDROL 125MG be taken with other medications?

Yes, SOLU-MEDROL can interact with other medications. It's crucial to inform your doctor or pharmacist about all prescription, over-the-counter, and herbal products you are using. Some products that may interact include:

  • Anticoagulants (blood thinners like warfarin), NSAIDs (like ibuprofen, aspirin)
  • Certain antibiotics (e.g., macrolides like erythromycin), antifungals (e.g., ketoconazole)
  • Immunosuppressants (e.g., cyclosporine)
  • Drugs for diabetes, estrogens, certain seizure medications, and St. John's wort.
  • Live vaccines are contraindicated for patients on immunosuppressive doses of corticosteroids.

What precautions should be taken with SOLU-MEDROL 125MG?

  • Vaccinations: Do not receive live vaccines while on immunosuppressive doses of SOLU-MEDROL.
  • Stress: Long-term corticosteroid use can affect your body's ability to respond to physical stress (e.g., surgery, injury). Inform healthcare professionals that you are using this medication.
  • Discontinuation: Do not stop using this medication suddenly without consulting your doctor, as it can lead to withdrawal symptoms. Your dose may need to be gradually decreased.
  • Allergies: Inform your doctor if you are allergic to methylprednisolone, prednisone, or any components of SOLU-MEDROL, including cow's milk proteins found in some formulations.

What happens if a dose is missed?
