HUMINSULIN N 100IU VAIL (COLD CHAIN)

Name:

  • Brand Name: HUMINSULIN N 100IU
  • Common Name: Isophane Insulin Human (NPH Insulin)

Composition: Each milliliter of HUMINSULIN N 100IU suspension contains 100 International Units (IU) of Human Insulin (rDNA origin) as Insulin Isophane. It also contains inactive ingredients such as m-cresol, glycerin, phenol, protamine sulfate, dibasic sodium phosphate, zinc oxide, and water for injection, with pH adjusted by hydrochloric acid and/or sodium hydroxide.

Uses: HUMINSULIN N 100IU is an intermediate-acting insulin prescribed to improve blood sugar control in both adults and children with:

  • Type 1 Diabetes Mellitus: Provides basal insulin coverage to manage blood glucose levels and prevent hyperglycemia.
  • Type 2 Diabetes Mellitus: Helps control blood sugar when diet, exercise, and oral medications are insufficient, thereby improving glycemic control and reducing diabetes complications.
  • It may also be used in gestational diabetes and for initial stabilization during severe stress, such as diabetic ketoacidosis, hyperosmolar non-ketotic syndrome, and major surgery.

Dosage and Administration: HUMINSULIN N 100IU is administered via subcutaneous injection (under the skin) into areas such as the thighs, abdomen, buttocks, or upper arm.

  • Preparation: Before each injection, the vial must be gently rolled 10 times and inverted 10 times to ensure the suspension is uniformly cloudy or milky. Do not use if it appears clear, contains particulate matter, or has clumps.
  • Site Rotation: To prevent lipodystrophy and localized skin irritation, rotate injection sites within the same region (e.g., abdomen) from one injection to the next.
  • Dosage: The dosage is individualized based on the patient's metabolic needs, blood glucose monitoring results, and glycemic control goals. It is typically injected once or twice daily.
  • Caution: This medication is generally not for intravenous or intramuscular administration, except in specific hospital settings under strict medical supervision for conditions like diabetic ketoacidosis.

Side Effects:

  • Common: Hypoglycemia (low blood sugar), injection site reactions (pain, redness, itching, swelling), weight gain, headache, edema (swelling of hands and feet), nervousness, tremor, confusion, unusual tiredness, nausea, and palpitations.
  • Serious: Lipodystrophy (abnormal changes in fat tissue at injection sites), severe hypersensitivity reactions (including anaphylaxis), hypokalemia (low blood potassium), and, rarely, worsening or development of heart failure when used with certain diabetes medications (thiazolidinediones).

Precautions:

  • Do not use if blood glucose levels are low (hypoglycemia).
  • Exercise caution in patients with kidney, liver, or heart problems; dosage adjustments may be necessary.
  • Alcohol consumption can increase the risk of both hypoglycemia and hyperglycemia.
  • Inform your doctor if you are pregnant or breastfeeding, as dosage requirements may change.
  • Changes in insulin strength, manufacturer, or type, as well as changes in physical activity, meal patterns, stress, or illness, may require dosage adjustments under medical supervision.
  • Never share needles, syringes, or insulin pens with other people.
  • Interactions with other medications (e.g., oral antidiabetic agents, corticosteroids, beta-blockers) may affect blood sugar levels, requiring dose adjustments and careful monitoring.

Storage: HUMINSULIN N 100IU is a cold chain medicine.

  • Unopened Vials: Store in a refrigerator between 2°C and 8°C (36°F and 46°F). Do not freeze.
  • Opened (In-use) Vials: Can be kept at room temperature, but below 30°C (86°F), away from direct heat and light, for up to 31 days.
  • Do not use if the medicine has been frozen or exposed to temperatures above 37°C (98.6°F).
  • Keep out of the sight and reach of children.
Frequently asked questions

Here are some frequently asked questions about HUMINSULIN N 100IU VAIL (Human Insulin NPH), along with concise and accurate answers:

What is HUMINSULIN N used for?

HUMINSULIN N 100IU VAIL is a type of intermediate-acting human insulin used to improve blood sugar control in both adults and children with Type 1 and Type 2 diabetes mellitus. It helps manage blood sugar levels between meals and overnight, thereby preventing complications of diabetes such as kidney damage, blindness, and nerve problems.

How does HUMINSULIN N work?

HUMINSULIN N, which contains Insulin NPH (Neutral Protamine Hagedorn), works by increasing insulin levels in your body to decrease high blood sugar. It helps glucose enter your muscle and fat cells to be used for energy and also reduces glucose production in the liver. The protamine in NPH insulin slows its absorption, leading to a sustained effect for 14 to 24 hours.

What is the usual dosage of HUMINSULIN N?

The dosage of HUMINSULIN N is highly individualized and determined by your doctor based on your blood sugar levels and metabolic needs. For adults, a common daily maintenance range is 0.5-1 unit/kg/day administered subcutaneously in divided doses. It is typically injected once or twice daily, 30-60 minutes before a meal. Always follow your doctor's specific instructions for dosing.

What are the possible side effects of HUMINSULIN N?

The most common side effect is low blood sugar (hypoglycemia), which can cause symptoms like headache, confusion, sweating, tremors, and dizziness. Other possible side effects include injection site reactions (redness, pain, swelling), weight gain, and lipodystrophy (thickening or hollowing of the skin at the injection site due to repeated injections in the same area). More serious, but less common, side effects can include low potassium levels (hypokalemia) and allergic reactions.

Is HUMINSULIN N safe during pregnancy or breastfeeding?

HUMINSULIN N is generally considered safe to use during pregnancy if prescribed by a doctor. Maintaining good blood glucose control is crucial during pregnancy, and insulin requirements may change. It is also safe to use while breastfeeding, and breastfeeding women may require lower doses of insulin. No adverse reactions have been reported in breastfed infants.

Can HUMINSULIN N be taken with other medications?

Many medications can interact with HUMINSULIN N, either increasing or decreasing its glucose-lowering effect. It has severe interactions with pramlintide and alcohol (ethanol). Other drugs like ACE inhibitors, ARBs, certain antibiotics (e.g., ciprofloxacin, levofloxacin), and high doses of salicylates (e.g., aspirin) can increase the risk of hypoglycemia. Conversely, corticosteroids, niacin, estrogens, oral contraceptives, and certain decongestants can decrease insulin's effect. Beta-blockers may also mask the symptoms of low blood sugar. Always inform your doctor about all prescription, over-the-counter medications, and supplements you are taking.

What precautions should be taken while using HUMINSULIN N?

It is crucial never to share insulin needles or pens with others to prevent the transmission of bloodborne illnesses. You should rotate your injection sites to avoid skin changes like lipodystrophy and regularly monitor your blood sugar levels as directed by your doctor. Always carry a quick source of sugar (like glucose tablets or juice) to treat potential low blood sugar episodes. Alcohol consumption should be avoided or approached with caution as it can increase the risk of severe hypoglycemia. Inform your doctor if you have kidney or liver problems, as dosage adjustments may be necessary. Do not use the medicine if it looks abnormal (e.g., contains particles) or if it has been frozen.

What happens if a dose of HUMINSULIN N is missed?

If you miss a dose of HUMINSULIN N, contact your doctor or pharmacist immediately for specific instructions. Do not take a double dose to make up for a missed one.

How should HUMINSULIN N be stored?

Unopened HUMINSULIN N vials should be stored in a refrigerator between 2°C and 8°C (36°F and 46°F) and protected from light until their expiration date. Do not freeze the medicine, and do not use it if it has been frozen. Once opened, vials can typically be stored in the refrigerator or at room temperature (below 30°C or 86°F, or 25°C or 77°F depending on the manufacturer's guidelines) for up to 28 to 31 days. Always discard opened vials after this period, even if some medicine remains.
