DUOLIN FORTE ROTACAPS

DUOLIN FORTE ROTACAPS, manufactured by Cipla Health Limited, is a combination medication designed to facilitate easier breathing for individuals with certain respiratory conditions.

Composition

DUOLIN FORTE ROTACAPS contains two active ingredients:

  • Levosalbutamol (100mcg)
  • Ipratropium Bromide (80mcg) These are bronchodilators that work synergistically to relax muscles in the airways and widen them.

Uses

DUOLIN FORTE ROTACAPS are primarily used for the treatment and prevention of symptoms associated with:

  • Chronic Obstructive Pulmonary Disease (COPD): This includes conditions like emphysema and chronic bronchitis, and it helps relieve symptoms such as shortness of breath, coughing, and wheezing.
  • Asthma: It helps in managing and preventing asthma symptoms by reducing airway swelling and widening the airways.
  • Bronchitis: It is beneficial for managing the inflammation of bronchial tubes.
  • Bronchospasm: It helps in treating or preventing bronchospasm in adults and adolescents 12 years of age and older with reversible obstructive airway disease.

Dosage and Administration

DUOLIN FORTE ROTACAPS are for inhalation only and should not be swallowed. The exact dose and duration will be determined by a doctor based on age, weight, and medical condition.

  • Typically, the capsule is inserted into a Rotahaler/Transhaler device.
  • After inserting, the mouthpiece is closed firmly, and the user breathes out fully.
  • The mouthpiece is then placed between the teeth, lips closed tightly, and the user breathes in rapidly and deeply, holding their breath for approximately 10 seconds or as comfortable.
  • After use, the empty rotacap is discarded.
  • Gargling with warm water after each inhalation is recommended to prevent fungal infections in the mouth and throat. This medicine does not work immediately and should not be used for sudden breathing problems; a rescue inhaler should be used for sudden difficulty in breathing.

Side Effects

Common side effects may include:

  • Headache
  • Dry mouth
  • Sore throat
  • Cough
  • Chest tightness
  • Dizziness
  • Irregular heartbeats or palpitations
  • Nervousness
  • Tremors, especially of hands
  • Nausea and vomiting
  • Runny or stuffy nose
  • Muscle cramps or pain

Serious side effects are less common but require immediate medical attention if experienced. These include signs of allergic reactions (skin rash, swelling, difficulty breathing), sudden worsening of shortness of breath, or significant changes in pulse rate or blood pressure.

Precautions

  • Allergies: Do not use if allergic to Levosalbutamol, Ipratropium, or any other components.
  • Underlying Conditions: Inform your doctor if you have high blood pressure, diabetes, epilepsy (fits), glaucoma, thyroid problems, heart problems (including heart disease, severe heart issues), kidney or liver diseases, an enlarged prostate, or bladder-neck obstruction.
  • Potassium Levels: DUOLIN FORTE ROTACAPS may cause a decrease in blood potassium levels (hypokalemia). Regular monitoring of potassium levels is recommended, especially during prolonged use.
  • Pregnancy and Breastfeeding: Generally not recommended during pregnancy or breastfeeding unless deemed necessary by a doctor, as safety data is limited.
  • Children and Elderly: Use with caution in children (safety below 12 years is unknown) and elderly patients, consulting a healthcare professional.
  • Driving/Operating Machinery: May cause blurred vision or dizziness, so caution is advised.
  • Alcohol: Avoid consuming alcohol as it may cause excessive drowsiness.
  • Overuse: Do not exceed the prescribed dosage, as this can lead to serious heart or lung issues.

Storage

  • Store in a cool and dry place, protected from moisture, sunlight, and heat, typically below 25°C or 30°C.
  • Keep DUOLIN FORTE ROTACAPS out of reach and sight of children and pets.
  • Keep the rotacaps in their original packaging until ready for use to protect them from moisture.
  • Avoid exposing the medication to open flame or heat.
Frequently asked questions

What is DUOLIN FORTE ROTACAPS used for?

DUOLIN FORTE ROTACAPS is primarily used for the treatment and prevention of symptoms associated with Chronic Obstructive Pulmonary Disease (COPD), including conditions like emphysema and chronic bronchitis. It is also effective in managing asthma and bronchitis. The medicine helps relieve symptoms such as coughing, wheezing, shortness of breath, and chest tightness.

How does DUOLIN FORTE ROTACAPS work?

DUOLIN FORTE ROTACAPS is a combination of two bronchodilators: Levosalbutamol and Ipratropium bromide. Levosalbutamol relaxes the smooth muscles in the bronchial tubes, while Ipratropium bromide, an anticholinergic, also helps relax airway muscles. Together, they work by relaxing the muscles in the airways of the lungs, widening them, and making it easier for air to get in and out, thereby improving breathing.

What is the usual dosage for DUOLIN FORTE ROTACAPS?

The dosage of DUOLIN FORTE ROTACAPS should be exactly as prescribed by your doctor. It is typically used once or twice daily. For COPD, a common recommendation is 1-2 puffs, up to 4 times daily. The rotacap is for inhalation only and should not be swallowed; it is administered using a Rotahaler device. The medicine usually starts working within 5 to 10 minutes, with effects lasting approximately 3 to 6 hours. It is important to note that this medicine is not for immediate relief of sudden breathing problems; a separate rescue inhaler should be used for acute attacks.

What are the possible side effects of DUOLIN FORTE ROTACAPS?

Common side effects may include headache, dry mouth, sore throat, cough (sometimes with mucus), chest tightness, dizziness, and irregular heartbeats. Other reported side effects include runny or stuffy nose, nervousness, tremors, nausea, vomiting, muscle cramps, back pain, fatigue, taste changes, and palpitations. More serious side effects, such as allergic reactions (skin rash, swelling, difficulty breathing, or significant changes in pulse/blood pressure), or signs of low potassium (irregular heartbeats, leg cramps, extreme thirst, weakness), require immediate medical attention. DUOLIN FORTE ROTACAPS can also cause a decrease in blood potassium levels and, in rare cases, paradoxical bronchospasm (sudden worsening of breathing).

Is DUOLIN FORTE ROTACAPS safe during pregnancy or breastfeeding?

DUOLIN FORTE ROTACAPS is generally not recommended during pregnancy unless absolutely necessary, as animal studies have shown potential adverse effects on the fetus, and human studies are limited. The benefits must be carefully weighed against the risks by a healthcare professional. For breastfeeding women, its safety is also not definitively established, and it is considered probably unsafe; consultation with a doctor is advised to determine if the benefits outweigh the potential risks to the baby.

Can DUOLIN FORTE ROTACAPS be taken with other medications?

It is crucial to inform your doctor about all other medications you are taking, including supplements and herbal products, to prevent potential interactions. DUOLIN FORTE ROTACAPS should be used with caution or avoided with certain medications, including:

  • Beta-blockers (e.g., propranolol, metoprolol): Can inhibit the effect of Duolin Forte.
  • Other anticholinergic agents (e.g., atropine, benztropine).
  • Diuretics (e.g., furosemide, hydrochlorothiazide): May worsen the risk of low potassium levels.
  • Digoxin (used for heart failure): Its levels may be decreased.
  • MAO inhibitors and tricyclic antidepressants (used for depression).
  • Other bronchodilators or adrenergic agonists (e.g., metaproterenol, epinephrine) due to increased risk of cardiovascular side effects.
  • Xanthine preparations (e.g., theophylline).

What precautions should be taken while using DUOLIN FORTE ROTACAPS?

Before starting this medicine, inform your doctor if you have any allergies or pre-existing medical conditions such as kidney or liver diseases, high blood pressure, diabetes, epilepsy, glaucoma, heart problems, an enlarged prostate, bladder obstruction, or thyroid disorders. Adhere strictly to the prescribed dosage, as exceeding it can lead to serious heart or lung complications. Regular monitoring of your blood potassium levels may be recommended during prolonged use. After each inhalation, gargle with warm water to help prevent oral fungal infections. The first dose should be taken under medical supervision due to the risk of immediate paradoxical bronchospasm. If you experience blurred vision or dizziness, avoid driving or operating machinery. This medicine is generally not recommended for children under 12 years of age and should be used with caution in elderly patients.

What happens if a dose of DUOLIN FORTE ROTACAPS is missed?

If you miss a dose of DUOLIN FORTE ROTACAPS, take it as soon as you remember. However, if it is almost time for your next scheduled dose, skip the missed dose and continue with your regular dosing schedule. Do not take a double dose to make up for the forgotten one, as this can be harmful.

How should DUOLIN FORTE ROTACAPS be stored?

DUOLIN FORTE ROTACAPS should be stored in a cool, dry place, away from direct sunlight, moisture, and heat. Keep the rotacaps in their original packaging until ready for use. Ensure the medicine is kept out of reach and sight of children and pets. The recommended storage temperature is typically between 10°C and 25°C, or below 30°C.
