Description
SIZOPIN 200 MG TABLET
Name:
- Brand Name: Sizopin 200 mg Tablet
- Common Name: Clozapine (active ingredient)
Composition:
- Active Ingredient: Clozapine 200 mg
Uses: SIZOPIN 200 MG TABLET is an atypical antipsychotic primarily used for:
- Managing schizophrenia, especially in individuals where at least two other antipsychotic medicines have been ineffective or caused severe side effects.
- Managing psychotic disorders associated with Parkinson's disease when standard management is ineffective.
- Reducing the risk of suicidal behavior in patients with schizophrenia or similar disorders.
Dosage and Administration: Take SIZOPIN 200 MG TABLET as advised by your physician, usually once or twice daily. It can be taken with or without food, but it is recommended to take it at the same time each day to maintain consistent medicine levels. Do not chew, crush, or break the tablet; swallow it whole with a glass of water, or if it's a mouth-dissolving tablet, allow it to dissolve completely in your mouth. Initial doses may be around 12.5 mg once or twice daily, gradually increasing by 25-50 mg/day over two weeks to a target dose of 300-450 mg/day. The maximum dosage for adults can be up to 900 mg. Do not discontinue the medicine suddenly, as it can lead to withdrawal symptoms or a relapse of symptoms. If a dose is missed, take it as soon as remembered, but if it's almost time for the next dose, skip the missed one.
Side Effects:
- Common Side Effects: Dry mouth, nausea, vomiting, loss of appetite, constipation, drowsiness, dizziness, increased saliva production, tiredness, headache, fever, sweating, trembling, tremor, stiffness of limbs, restlessness, problems in coordination, slurring of words, abnormal movements, weight gain, blurred vision, fast heartbeat (tachycardia), and low blood pressure.
- Serious Side Effects: Severe constipation, signs of a cold/fever/flu-like symptoms/sore throat (indicating a possible decrease in white blood cells like agranulocytosis), seizures, sudden fainting/loss of consciousness with muscle weakness, and signs of neuroleptic malignant syndrome (e.g., sudden rapid increase in body temperature, rigid muscles).
Precautions:
- Contraindications: Do not take if allergic to clozapine or any inactive ingredients, or if you have uncontrolled fits/epilepsy, reduced consciousness, severe drowsiness, certain brain disorders, severe kidney or liver disease (especially with jaundice), or certain heart diseases (e.g., myocarditis, pericarditis, cardiomyopathy), or a low white blood cell count.
- Warnings: Use with caution during pregnancy, only if clearly necessary, and it is generally not recommended during breastfeeding as it may pass into breastmilk. Avoid driving or operating machinery if you experience dizziness, tiredness, or drowsiness. Avoid alcohol consumption as it can increase drowsiness and other unwanted effects. Regular blood tests are often required due to the risk of a serious side effect called agranulocytosis (a decrease in white blood cells). Monitor blood sugar, cholesterol, and body weight regularly.
- Interactions: Inform your doctor about all medications, including over-the-counter drugs and herbal supplements, due to potential drug interactions. Avoid consuming grapefruit or grapefruit juice as it may increase clozapine levels.
Storage: Store SIZOPIN 200 MG TABLET below 30°C, away from direct sunlight, in a cool, dark place without humidity. Protect from light and moisture.
Frequently asked questions
What is SIZOPIN 200 MG TABLET used for?
SIZOPIN 200 MG TABLET contains clozapine, an atypical antipsychotic medicine used to treat severely ill adult patients with schizophrenia who have not responded adequately to other antipsychotic treatments or who cannot tolerate other medications due to severe side effects. It is also used to reduce the risk of suicidal behavior in patients with schizophrenia or schizoaffective disorder. Additionally, it may be prescribed for psychotic disorders in patients with Parkinson's disease when standard treatments are ineffective.
How does SIZOPIN 200 MG TABLET work?
Clozapine, the active ingredient in SIZOPIN 200 MG TABLET, works by balancing certain natural chemical substances (neurotransmitters) in the brain. It interacts with various receptors, including dopamine (D2) and serotonin (5-HT2A) receptors, to help reduce symptoms like hallucinations, delusions, and thought disturbances associated with schizophrenia.
What is the usual dosage for SIZOPIN 200 MG TABLET?
The dosage of SIZOPIN 200 MG TABLET is highly individualized and determined by your doctor. For schizophrenia, treatment typically starts with a low dose, such as 12.5 mg once or twice a day. The dose is then gradually increased, often by 25-50 mg daily, to a target dose of 300-450 mg per day by the end of two weeks, administered in divided doses. The maximum daily dose is usually not more than 900 mg. For psychotic disorders in Parkinson's disease, a lower starting dose, often 12.5 mg at bedtime, is used, with a maximum of 100 mg daily.
What are the possible side effects of SIZOPIN 200 MG TABLET?
SIZOPIN 200 MG TABLET can cause various side effects, some common and others serious. Common side effects include dry mouth, nausea, vomiting, constipation, drowsiness, dizziness, increased salivation, headache, weight gain, blurred vision, fast heart rate, and tremors.
More serious side effects can include a potentially life-threatening decrease in white blood cells (agranulocytosis), seizures, inflammation of the heart muscle (myocarditis), low blood pressure when standing up (orthostatic hypotension), and severe constipation that can lead to serious bowel problems. Your doctor will require regular blood tests to monitor for these serious side effects.
Is SIZOPIN 200 MG TABLET safe during pregnancy or breastfeeding?
Pregnancy: SIZOPIN 200 MG TABLET should be used during pregnancy only if clearly necessary and after consulting your doctor to weigh the potential benefits against the risks. While animal studies have shown low or no adverse effects, human studies are limited. Clozapine crosses the placenta and may increase the risk of gestational diabetes.
Breastfeeding: It is generally not recommended to breastfeed while taking SIZOPIN 200 MG TABLET. Clozapine can pass into breast milk and may harm the baby, potentially increasing the risk of serious side effects like agranulocytosis in the infant.
Can SIZOPIN 200 MG TABLET be taken with other medications?
SIZOPIN 200 MG TABLET can interact with many other medications. It's crucial to inform your doctor about all medicines you are taking, including over-the-counter drugs and herbal supplements. Concomitant use with certain medications, such as strong CYP1A2 inhibitors (e.g., fluvoxamine, ciprofloxacin) may require a dose reduction of clozapine. Strong CYP3A4 inducers (e.g., carbamazepine, phenytoin, rifampicin) are generally not recommended. Taking it with alcohol or other central nervous system depressants can worsen side effects. Additionally, it may affect the effectiveness of antidiabetic medications and can increase hypotensive symptoms if taken with antihypertensives.
What precautions should be taken while taking SIZOPIN 200 MG TABLET?
Regular blood tests are essential to monitor your white blood cell count due to the risk of agranulocytosis. Do not stop taking SIZOPIN 200 MG TABLET suddenly, as this can lead to withdrawal symptoms or a worsening of your condition; any discontinuation should be gradual and under medical supervision. Be aware of potential dizziness or lightheadedness, especially when standing up, and rise slowly. Report any signs of infection (like fever or sore throat) or severe constipation immediately to your doctor. Avoid alcohol consumption. Due to potential drowsiness and blurred vision, do not drive or operate heavy machinery until you know how this medicine affects you.
What happens if a dose of SIZOPIN 200 MG TABLET is missed?
If you miss a dose of SIZOPIN 200 MG TABLET, take it as soon as you remember. However, if it is almost time for your next scheduled dose, skip the missed dose and continue with your regular dosing schedule. Do not take a double dose to make up for a missed one. If you miss taking clozapine for two or more consecutive days (or more than 48 hours), you must contact your doctor immediately before restarting the medication, as you may need to restart at a lower dose and have it gradually re-titrated.
How should SIZOPIN 200 MG TABLET be stored?
SIZOPIN 200 MG TABLET should be stored in a closed container at room temperature, typically between 20-25°C (68-77°F) or below 30°C. Keep the medicine away from heat, moisture, and direct light. Do not refrigerate or freeze it. Keep all medications out of the sight and reach of children. Orally disintegrating tablets should remain in their original package until you are ready to use them to protect them from moisture.
