Description
CORALAN 7.5 MG TABLET
Coralan 7.5 mg Tablet is a medication prescribed for the management of certain heart conditions, primarily by regulating heart rate.
Name
- Common Name: Ivabradine
- Brand Name: CORALAN
Composition Each CORALAN 7.5 mg film-coated tablet contains 7.5 mg of ivabradine (as the hydrochloride salt). Inactive ingredients include lactose monohydrate, magnesium stearate, maize starch, maltodextrin, colloidal anhydrous silica, hypromellose, titanium dioxide (E 171), macrogol 6000, glycerol, yellow iron oxide (E 172), and red iron oxide (E 172).
Uses CORALAN 7.5 mg Tablet is used for:
- Symptomatic treatment of chronic stable angina pectoris: This refers to chest pain caused by insufficient blood flow to the heart, particularly in adult patients with coronary artery disease, normal sinus rhythm, and a resting heart rate of 70 beats per minute (bpm) or higher. It helps to reduce the frequency of angina attacks and improve exercise tolerance.
- Chronic heart failure: It is prescribed to adults with chronic heart failure to improve heart function, reduce symptoms like fatigue and breathlessness, and lower the risk of hospitalisation due to worsening heart conditions.
Dosage and Administration
- Coralan tablets should be taken orally twice daily, once in the morning and once in the evening, during meals.
- Swallow the tablet whole with a glass of water; do not crush, break, or chew it.
- For Stable Angina: The typical starting dose should not exceed one CORALAN 5 mg tablet twice a day. After three to four weeks, the dose may be adjusted by your doctor, not exceeding a maintenance dose of one CORALAN 7.5 mg tablet twice daily, depending on symptom control and resting heart rate.
- For Heart Failure: The usual starting dose is one CORALAN 5 mg tablet twice a day. After two weeks, the dose may be increased to one CORALAN 7.5 mg tablet twice daily if the resting heart rate is persistently above 60 bpm, or decreased based on the heart rate and symptoms.
Side Effects
- Common Side Effects: Temporary brightness/flashes in vision (luminous phenomena/phosphenes), slow heart rate (bradycardia), headache, dizziness, blurred vision, high blood pressure, nausea, abdominal pain, diarrhea, and ECG changes.
- Serious Side Effects: These can include irregular or rapid heartbeat (atrial fibrillation), significant changes to your heartbeat, and fainting. Seek immediate medical attention if these occur.
Precautions
- Contraindications: Do not take CORALAN if you are allergic to ivabradine or any other components, have a resting heart rate below 70 bpm prior to treatment, cardiogenic shock, acute myocardial infarction, severe hypotension, severe liver disease, sick sinus syndrome, sino-atrial block, unstable or acute heart failure, are pacemaker-dependent, have unstable angina, or a 3rd-degree AV block.
- Pregnancy and Breastfeeding: CORALAN is not recommended during pregnancy as it may harm the unborn baby and is unsafe during breastfeeding.
- Drug Interactions: Avoid grapefruit juice while taking CORALAN. Concomitant use with strong CYP3A4 inhibitors (e.g., ketoconazole, clarithromycin, ritonavir, diltiazem, verapamil) is contraindicated. Caution is advised with CYP3A4 inducers (e.g., rifampicin, phenytoin, St. John's wort), as they may decrease ivabradine exposure.
- Other Warnings: Exercise caution when driving or operating machinery due to potential dizziness or blurred vision. It is not recommended for use in children and adolescents younger than 18 years.
Storage Keep CORALAN tablets in a cool, dry place where the temperature stays below 30°C. Protect from moisture and direct sunlight. Do not store in the fridge or freezer, in a bathroom, or near a sink. Keep the tablets in their original blister pack and out of reach of children.
Frequently asked questions
CORALAN 7.5 MG TABLET: Frequently Asked Questions
What is CORALAN 7.5 MG TABLET used for?
CORALAN 7.5 MG TABLET is primarily used to treat chronic stable angina (chest pain) and chronic heart failure. It helps to manage symptoms like chest pain, reduce the frequency of angina attacks, and decrease the risk of hospitalization for heart-related issues by improving heart function and exercise tolerance. It is used in patients who cannot tolerate beta-blockers or when beta-blocker therapy is contraindicated or not tolerated.
How does CORALAN 7.5 MG TABLET work?
CORALAN 7.5 MG TABLET contains ivabradine, which is a heart-rate-lowering agent. It works by selectively inhibiting the cardiac pacemaker If current in the sinus node, which controls the heart's natural rhythm. By slowing the heart rate, it allows the heart to pump more blood with each beat, reduces the heart's workload and oxygen demand, and improves blood flow to the heart muscle.
What is the usual dosage for CORALAN 7.5 MG TABLET?
The dosage of CORALAN 7.5 MG TABLET is determined and adjusted by your doctor based on your condition and response to treatment. For stable angina, the usual starting dose is one CORALAN 5 mg tablet taken twice a day, which may be increased to a maintenance dose of one CORALAN 7.5 mg tablet twice daily if needed, after consulting your doctor. For heart failure, the usual starting dose is one CORALAN 5 mg tablet twice a day, and after two weeks, it may be adjusted up to a maximum of one CORALAN 7.5 mg tablet twice daily. The tablets should be taken orally twice daily, once in the morning and once in the evening, with food.
What are the possible side effects of CORALAN 7.5 MG TABLET?
Common side effects include luminous visual phenomena (temporary brightness or flashes in vision, halos, or colored flashes, especially with sudden changes in light intensity), slow heart rate (bradycardia), headache, dizziness, and blurred vision. Other possible side effects include nausea, abdominal pain, diarrhea, and high blood pressure. Serious side effects, though less common, can include irregular or rapid heartbeats (atrial fibrillation) and very slow heart rate. If you experience persistent or worsening side effects, or symptoms like difficulty breathing, swelling of the face, lips, mouth, tongue or throat, or changes in heart rate, seek immediate medical attention.
Is CORALAN 7.5 MG TABLET safe during pregnancy or breastfeeding?
No, CORALAN 7.5 MG TABLET is not recommended during pregnancy as it may harm the unborn baby. Women of childbearing potential should use appropriate contraceptive measures during treatment. It is also not recommended during breastfeeding, and breastfeeding should be discontinued if you are taking CORALAN.
Can CORALAN 7.5 MG TABLET be taken with other medications?
CORALAN 7.5 MG TABLET can interact with certain medications. It is contraindicated with potent CYP3A4 inhibitors such as azole antifungals (e.g., ketoconazole, itraconazole), macrolide antibiotics (e.g., clarithromycin, erythromycin), HIV protease inhibitors (e.g., nelfinavir, ritonavir), and nefazodone. Concomitant use with heart rate-reducing calcium channel blockers like verapamil or diltiazem is also contraindicated due to the risk of additive heart rate lowering effects. Caution should be exercised with moderate CYP3A4 inhibitors (e.g., fluconazole), CYP3A4 inducers (e.g., rifampicin, barbiturates, phenytoin, St. John's Wort), and certain diuretics that can cause a decrease in blood potassium level. Always inform your doctor about all medications, herbal supplements, and vitamins you are taking to avoid potential interactions.
What precautions should be taken when taking CORALAN 7.5 MG TABLET?
Avoid grapefruit and grapefruit juice while taking CORALAN. If you have heart rhythm disorders, a slow heartbeat (less than 70 bpm before treatment), severe hypotension, severe liver or kidney problems, or a recent stroke, discuss these with your doctor. CORALAN is not recommended for individuals under 18 years of age. Due to potential dizziness or blurred vision, be cautious when driving or operating machinery, especially in situations with sudden changes in light intensity or at night.
What happens if a dose is missed?
If you forget to take a dose of CORALAN 7.5 MG TABLET, take the next dose at your usual time. Do not take a double dose to make up for the missed one.
How should CORALAN 7.5 MG TABLET be stored?
Store CORALAN 7.5 MG TABLET in a cool, dry place where the temperature stays below 30 degrees C. Do not store it in the fridge or freezer, or in a bathroom or near a sink. Keep the medicine in its original blister pack until it is time to take it and out of reach of children.
