  • Severe Epileptic Seizures/Fits (Status Epilepticus): It is used for the rapid and effective control of generalized tonic-clonic status epilepticus.
  • Prevention of Seizures: Used to prevent seizures during or after severe head injury and/or neurosurgery.
  • Cardiac Arrhythmias: It may also be used to treat certain cardiac arrhythmias (irregular heartbeat) caused by specific medicines or in patients unresponsive to conventional anti-arrhythmic treatments.

Dosage and Administration

Eptoin Injection is administered by a doctor or healthcare professional, typically intravenously. Self-administration is not recommended.

  • Loading Dose for Status Epilepticus (Adults): A loading dose of 10-15 mg/kg should be administered slowly intravenously, not exceeding 50 mg per minute.
  • Loading Dose for Status Epilepticus (Pediatric): A loading dose of 15-20 mg/kg intravenously, at a rate not exceeding 1-3 mg/kg/min or 50 mg per minute, whichever is slower.
  • Maintenance Dose (Adults): Typically 100 mg intravenously or orally every 6 to 8 hours following the loading dose.
  • Neurosurgery (Prophylactic): 100-200 mg intramuscularly at approximately 4-hour intervals during surgery and post-operative period for 48-72 hours, then reduced to a maintenance dose. Intramuscular injections should not be continued for more than one week.
  • Important Note: The injection should be diluted in normal saline, and the final concentration should not exceed 10 mg/ml. • Megaloblastic anemia
  • Suicidal thoughts

Precautions

  • Allergies: Do not use if allergic to phenytoin or any of its contents.
  • Cardiovascular Conditions: Use with caution in patients with decreased heart rate, heart block, low blood pressure, heart failure, and other cardiac rhythm problems. Rapid IV administration can lead to cardiovascular toxicity.
  • Liver and Kidney Disease: Use with caution in patients with liver or kidney impairment, as dose adjustment and close monitoring may be required.
  • Pregnancy and Breastfeeding: Generally not recommended during pregnancy due to potential risks to the fetus. Avoid breastfeeding while on treatment.
  • Alcohol Consumption: Avoid alcohol as it may increase dizziness/drowsiness and affect phenytoin concentration.
  • Driving and Machinery: Avoid driving or operating heavy machinery as it may cause drowsiness and dizziness.
  • Diabetes: Use with caution in patients with diabetes mellitus due to the risk of elevated blood sugar levels.
  • Porphyria: Use with caution in patients with porphyria (blood disorder) as it may worsen the condition.

Frequently asked questions

EPTOIN INJ, containing phenytoin, is an anticonvulsant medication primarily used in hospital settings to manage and prevent seizures. It is administered when oral medication is not feasible.

Here are some frequently asked questions about Eptoin Injection:

What is Eptoin Injection used for?

Eptoin Injection is used to treat and prevent various types of seizures, including generalized convulsive status epilepticus (prolonged seizures) and seizures occurring during or after neurosurgery or head injury. It may also be used to treat certain irregular heartbeats (cardiac arrhythmias) in specific situations.

How does Eptoin Injection work?

Eptoin Injection contains phenytoin, an antiepileptic medicine. It works by stabilizing neuronal membranes and decreasing the abnormal and excessive electrical activity of nerve cells in the brain, thereby preventing the spread of seizure activity.

What is the usual dosage of Eptoin Injection?

Eptoin Injection is administered by a doctor or nurse in a hospital or clinic setting. The dosage varies depending on the condition being treated and the patient's individual needs. For adults, a common loading dose for status epilepticus is 10 to 15 mg/kg, administered slowly intravenously at a rate not exceeding 50 mg per minute. This is typically followed by maintenance doses. Dosage in pediatric patients is also carefully calculated based on weight and condition.

What are the possible side effects of Eptoin Injection?

Common side effects include dizziness, drowsiness, nausea, vomiting, headache, slurred speech, decreased coordination, and uncontrolled eye movements. Long-term use can sometimes lead to gum overgrowth (gingival hyperplasia), peripheral neuropathy, and softening of bones (osteomalacia). Serious side effects, though rare, can include severe skin reactions (like Stevens-Johnson syndrome), liver problems, and cardiovascular issues such as low blood pressure or irregular heart rhythms, especially with rapid intravenous administration.

Is Eptoin Injection safe during pregnancy or breastfeeding?

Eptoin Injection is generally not recommended during pregnancy due to potential risks to the unborn baby, including congenital abnormalities. It should be used during pregnancy only if the potential benefits clearly outweigh the risks, and under strict medical supervision. Phenytoin, the active ingredient, passes into breast milk. Therefore, it is generally not recommended for breastfeeding mothers, or caution is advised. Your doctor will weigh the potential benefits against the risks.

Can Eptoin Injection be taken with other medications?

Eptoin Injection can interact with numerous other medications, potentially altering its effectiveness or increasing the risk of side effects. It's crucial to inform your doctor about all medicines, supplements, and herbal preparations you are taking. For example, it should not be taken with the antiviral medicine delavirdine. Other medications like certain antifungals, antibiotics, antidepressants, and blood thinners can increase or decrease phenytoin levels, or phenytoin can affect their efficacy.

What precautions should be taken with Eptoin Injection?

Inform your doctor if you have any heart disorders (including low blood pressure, heart failure, or heart rhythm problems), liver diseases, kidney disorders, or diabetes. Avoid consuming alcohol, as it can interfere with phenytoin levels and increase side effects. Since Eptoin Injection can cause dizziness and drowsiness, you should avoid driving or operating heavy machinery until you know how the medicine affects you. Do not stop taking this medication suddenly without consulting your doctor, as this can lead to an increase in seizure frequency.

What happens if a dose of Eptoin Injection is missed?

Missing a dose of Eptoin Injection is rare as it is administered by healthcare professionals in a hospital or clinic and you will be closely monitored. If you believe a dose was missed, inform your doctor or nurse immediately. Never take a double dose to make up for a missed one.

How should Eptoin Injection be stored?

Eptoin Injection should be stored at room temperature, in a cool and dry place, away from direct sunlight and moisture. It should be kept out of reach of children and pets. Any unused portion should be discarded properly. Do not use the injection if you notice crystals or deposits in the ampule.
