TOPLAP BIG 30GM

Toplap Big 30gm is a topical local anesthetic gel manufactured by Torrent Pharmaceuticals Ltd.

Composition Toplap Big 30gm contains a combination of two active ingredients:

  • Lidocaine (2.5% w/w)
  • Prilocaine (2.5% w/w)

These active ingredients work together to block pain signals from the nerves to the brain, thereby temporarily numbing the applied area and decreasing the sensation of pain.

Uses Toplap Big 30gm is primarily used to produce local anesthesia on intact skin before various medical and cosmetic procedures, including:

  • Needle insertion (e.g., intravenous catheter insertion, blood sampling, vaccinations)
  • Minor surgical procedures (e.g., removal of warts, moles, superficial skin operations)
  • Laser procedures (e.g., laser hair removal)
  • Tattooing and piercing
  • Topical anesthesia of leg ulcers for cleansing or debridement
  • Numbing the skin or surfaces of the penis or vagina for minor surgery or medical procedures.

Dosage and Administration Toplap Big 30gm is for external use only.

  • Apply the gel exactly as described by your doctor, pharmacist, or nurse.
  • For surgical procedures, it is often applied under medical supervision.
  • For minor procedures (like needle insertion), it may be applied at least 1 hour before. For more painful procedures, a gap of at least 2 hours before the procedure might be advised.
  • Apply a thick, even layer of the gel to the affected area and, if recommended, cover with an occlusive dressing.
  • Do not apply unnecessarily on broken or damaged skin, skin rash, cuts, grazes, or other open wounds (except leg ulcers).
  • Avoid getting the gel in your eyes, near the nose, mouth, ears, or anus.

Side Effects Common side effects at the application site include:

  • Burning sensation
  • Itching
  • Redness
  • Rashes
  • Pale skin or blanching
  • Swelling
  • Localized hyperpigmentation.

Serious side effects requiring immediate medical attention are rare but can include severe dizziness, breathing difficulty, or allergic reactions such as skin rash, hives, itching, or swelling of the face, tongue, or throat.

Precautions

  • Allergies: Do not use if you are allergic to Lidocaine, Prilocaine, or any other components of the gel.
  • Children: Do not use in children under 1 month of age. Use in children should be under medical supervision.
  • Specific Conditions: Exercise caution and inform your doctor if you or your child have a blood pigment disorder (methemoglobinemia), glucose-6-phosphate dehydrogenase deficiency, heart rhythm disorders, severe liver disease, pseudocholinesterase deficiency, cardiac conduction disturbances, or respiratory depression.
  • Pregnancy and Breastfeeding: Consult your doctor before using if you are pregnant, think you may be pregnant, are planning a baby, or are breastfeeding. It should be used during pregnancy only if clearly needed and with caution during breastfeeding.
  • Drug Interactions: Inform your doctor about all other medicines, supplements, or herbal products you are using, especially other local anesthetics, anti-infectives, anti-epileptics, medicines for uneven heartbeats, or stomach acid reducers. Toplap Gel should not be applied simultaneously with other topical medicines.

Frequently asked questions

Here are the most frequently asked questions about TOPLAP BIG 30GM (Lidocaine and Prilocaine) along with their answers:

What is TOPLAP BIG 30GM used for?

TOPLAP BIG 30GM is a local anesthetic gel or cream used to numb intact skin before various medical and cosmetic procedures, such as needle insertion (e.g., injections, blood draws), minor superficial surgical procedures (e.g., wart or mole removal), laser hair removal, tattooing, and piercing. It can also be used to numb the skin or surfaces of the penis or vagina for minor surgeries or medical procedures. In some cases, it may be used for pain associated with mucositis or to manage premature ejaculation.

How does TOPLAP BIG 30GM work?

TOPLAP BIG 30GM works through the combined action of its active ingredients, Lidocaine and Prilocaine, which are amide-type local anesthetics. It blocks pain signals from the nerves in the applied area from reaching the brain, thereby temporarily decreasing the sensation of pain and producing a numbing effect on the skin. This is achieved by blocking the influx of sodium ions into the nerve membrane, preventing the initiation and conduction of impulses.

What is the usual dosage and how should it be applied?

TOPLAP BIG 30GM is for external use only and should be applied exactly as prescribed by your doctor, pharmacist, or nurse. For minor procedures like needle insertion, typically apply about 2.5 g of the gel to a 20-25 cm² skin area, usually 1 hour before the procedure. For more painful procedures, the application time might need to be at least 2 hours. After cleaning and drying the affected area, apply a thick, even layer (about 2-3 mm high) of the gel and cover it with an occlusive dressing (a special film or patch), if recommended. Do not rub the gel into the affected area. The maximum recommended application area for adults is 60 cm², and the total dose should not exceed 10 g. Wash your hands thoroughly after applying the gel unless your hands are the treated area.

What are the possible side effects of TOPLAP BIG 30GM?

Common side effects of TOPLAP BIG 30GM include application site reactions such as burning, itching, redness, irritation, pale skin, or swelling. Other reported side effects can include rashes and changes in skin color at the application site. Most of these effects are usually temporary and resolve on their own. Rarely, systemic side effects like dizziness, drowsiness, blurred vision, slurred speech, or headache may occur.

Is TOPLAP BIG 30GM safe during pregnancy or breastfeeding?

Pregnancy: If you are pregnant, think you may be pregnant, or are planning a baby, consult your doctor before using TOPLAP BIG 30GM. It is generally considered safe for occasional use during pregnancy, with animal studies showing low or no adverse effects on the fetus. However, it should be used only if clearly needed and avoided on larger skin areas for prolonged durations.

Breastfeeding: Components of TOPLAP BIG 30GM can be absorbed through the skin and may pass into breast milk. However, the quantity is usually too small to affect the breastfed baby. It should be used with caution and after consulting your doctor. Always wash your hands after application and ensure the cream does not get onto your breast or nipple area before feeding.

Can TOPLAP BIG 30GM be taken with other medications?

TOPLAP BIG 30GM should not be applied simultaneously with other medicines that are to be applied locally on the same area to avoid excessive absorption and potential systemic side effects. Inform your doctor about all medicines and supplements you are taking, especially if you are using other local anesthetics, certain anti-infectives, anti-epileptics, medicines to treat uneven heartbeats (antiarrhythmic drugs or beta-adrenergic blockers), or stomach acid reducers. Concomitant use with other drugs structurally related to amide-type local anesthetics may lead to additive systemic toxic effects.

What precautions should be taken?

Do not apply TOPLAP BIG 30GM to areas with skin rash, cuts, grazes, broken or damaged skin, open wounds (except for leg ulcers if specifically directed by a doctor), or mucous membranes (except for genital application as directed). Avoid contact with eyes, nose, mouth, ears, or the anal area. If accidental eye contact occurs, rinse immediately with lukewarm water or saline and protect the eye until sensation returns. Inform your doctor if you or your child have a blood pigment disorder called methemoglobinemia, glucose-6-phosphate dehydrogenase deficiency, heart rhythm disorders, severe liver disease, or atopic dermatitis. It should not be used in infants under one month of age, or in infants under 12 months receiving methemoglobinemia-inducing agents. Use with caution in acutely ill, debilitated, and elderly patients.

What happens if a dose is missed?

Missing a dose of TOPLAP BIG 30GM does not have a significant effect as it is typically used only when local anesthesia is desired for a procedure. Since it's often applied before or during a medical procedure, the likelihood of a missed dose is very low. If you are applying it at home, use it as soon as you remember if it's not almost time for your next scheduled application. Do not apply a double dose.

How should it be stored?

Store TOPLAP BIG 30GM below 30°C, in a cool, dry place, away from heat sources and direct sunlight. Keep it out of the reach and sight of children. Do not freeze the gel. Avoid refrigerating it, as extreme temperatures can affect its stability and effectiveness. Medicines must not be used past their expiry date.
